To see our doctor for an appointment just follow these 3 easy steps


Step 1

Download and fill out a new patient application, fax it and your current medical records signed by a physician licensed MD or DO to 360-943-7441 for our review. We will contact you shortly to let you know we received your paperwork. if you don't hear from us within 1 week of faxing your paperwork please call 360-951-9668 to talk to a staff member.
 

Step 2

Once we have reviewed your medical records and determined that you have a qualifying condition, American Heritage Medical staff will contact you to set up an appointment time and date.

[Please allow 7 to 14 days].
 

Step 3

Come to your appointment and receive your authorization to posses, use, and grow cannabis [marijuana] for medical purposes.
