Only legal Washington state medical marijuana patients can become members of the Olympia Patient Resource Center
There are 2 Memberships available to patients a $25 Basic membership and a $40 ID card membership both carry the same benefits:

To become a member
The person seeking membership with the Olympia Patient Resource Center must, come to our office in person and provide a Washington State Drivers license or ID card, and a signed statement from a physician authorizing them to possess marijuana for medical purposes.
Upon acceptance of your authorization, the prospective member will be asked to fill out a membership application and an Authorization to disclose confidential health information.
When we receive completed form(s), we will verify the physician's approval by fax, which can vary depending on doctor response time.
Upon receipt of verification from your physician a member card will be issued to the patient.
